Please read and acknowledge the commitment required by you to proceed.

  • I will dedicate the time and effort to be successful in my completion of the test prep on my road to certification. I recognize this could range from 15 to 30 hours outside of class time.

  • I will NOT and am NOT authorized to print any materials within the test prep and must use all materials on line.

  • I will not take short cuts when taking the quizzes, by selecting arbitrary answers to get the correct answer. This has been tried and proven to be ineffective.

  • I will review the material that I have gotten incorrect and re-take the quiz until I achieve a successful result of 85% or higher on each quiz. I understand that I can take the quizzes multiple times.

  • I understand that the exam voucher will not be provided until I complete all practice quizzes and comprehensive exams with a score of 85% or higher.

  • I understand that I have 45 days from this date to complete the practice exam at the appropriate score. If I have not completed during that time, I must resubmit the request. When a resubmit occurs, my prior quiz scores are not retained and would require me to start fresh. After two requests, if I have not successfully completed the certification exam prep at the level needed to receive my exam voucher, I will not be eligible for the exam voucher for that particular certification.

  • REQUEST SUBMISSIONS MUST OCCUR WITHIN ONE WEEK OF WHEN YOU INTEND TO TAKE YOUR EXAM.

    DO NOT SUBMIT THIS FORM IF YOU DO NOT INTEND TO TAKE YOUR EXAM WITHIN 7 DAYS.
